This ground-breaking procedure represents a significant milestone in the city’s medical history and showcases the importance of timely intervention and awareness of available treatments.
HOCM, previously known as idiopathic hypertrophic subaortic stenosis, is a relatively common genetic disorder that can cause sudden cardiac death, particularly in young individuals and well-trained athletes. The condition is characterized by asymmetric septal hypertrophy, resulting in the obstruction of the left ventricle. While HOCM affects both men and women across all races, females tend to be more symptomatic, presenting at a younger age and experiencing more disability.
The patient, a 28-year-old woman, presented with a history of difficulty in breathing, recurrent syncopal attacks, and palpitations. Upon evaluation, she was diagnosed with severe symptomatic obstructive HOCM, with a gradient exceeding 80, a leaking mitral valve, and arrhythmia. Given the severity of her condition, urgent surgical correction was advised.
Under the expertise of our medical team, a complicated resection, plication, and release (RPR) technique, along with extended myomectomy, was performed on the patient’s overgrown heart muscle. The procedure was conducted using a heart-lung machine, ensuring a controlled environment. Additionally, the mitral valve was repaired to address the leakage.

During the one month follow-up, she remained completely asymptomatic, with no obstruction across the left ventricle and no leakage in the mitral valve. This case is likely the city’s first successful surgical repair for HOCM, highlighting the potential to save numerous lives through timely intervention and raising awareness of available treatments.
Hypertrophic obstructive cardiomyopathy affects approximately 1 in 500 individuals in the general adult population. In the United States alone, fewer than 100 deaths occur annually due to HOCM, with a rate of 1 in 220,000 among athletes. Early detection, proper diagnosis, and appropriate treatment can significantly improve outcomes for individuals affected by this condition.
