TIL Desk Lucknow:Doctors at Max Super Speciality Hospital, Lucknow, have successfully performed Uttar Pradesh’s first simultaneous Double Valve-in-Valve (VIV) procedure in an elderly patient in aortic and mitral positions. This highly complex, minimally invasive intervention offers a vital alternative to high-risk open-heart surgery for patients with severe valve degeneration.
The 77-year-old patient had undergone double valve replacement more than a decade ago. Over time, both valves had degenerated, leading to severe narrowing and leakage, resulting in life-threatening complications such as heart failure, pulmonary edema, and atrial fibrillation. The patient came with repeated admissions for heart failure & Pulmonary edema to the Max Hospital, Lucknow with symptoms including fatigue, shortness of breath and chest pain. Due to multiple comorbidities—including COPD and post-surgical adhesions—traditional re-do surgery was deemed too risky.
After a thorough evaluation, the medical team led by Dr Nakul Sinha, Chairman – Cardiology, Max Super Speciality Hospital, Lucknow, suggested a double-valve-in-valve procedure as the safest and most effective option. Valve In Valve Procedure(VIV) Transcatheter Aortic Valve Replacement (TAVR) and Transcatheter Mitral Valve Replacement (TMVR) procedure was recommended to the patient to replace the valves.
Under the guidance of Dr. Nakul Sinha, the skilled clinical team at Max Hospital, Lucknow replaced the two damaged heart valves using a minimally invasive approach—one in the aorta and another in the mitral position. To ensure the new valves functioned properly, the old ones were carefully widened using a special balloon technique. The procedure was completed smoothly, with excellent results—no major complications, no leaks, and proper blood flow.
